Does the breather hardware set include seals or a breather hose?

No. Seals, grommets and the breather hose are not included. Inspect them and the cam-cover threads and replace any hardened, damaged or missing parts separately.

What should be checked after installing the breather hardware?

Confirm the breather passage and hose are open, route the hose without kinks and tighten the hardware only to the specified torque. Check for oil seepage and abnormal crankcase pressure after startup.
